Guam Community Health Center
Guam CHC is located on the island of Guam, which is an unincorporated organized territory of the US. Guam CHC has two sites, one in the Northern region and one in the Southern region. The Guam Southern Region Community Health Center was first opened in 1984. Another service delivery site, the Northern Region Community Health Center opened in 1998. The CHC is a collaboration between the Guam Bureau of Primary Care Services and the Guam CHC Governing Board. Guam CHC is a Patient Centered Medical Home recognized by the National Committee for Quality Assurance.
Mission
The mission of the BPCS is to improve the health status of the people ofGuamand provide leadership in health information and surveillance, assurance of a healthful environment, and promotion of community partnerships.
Vision
The vision of the Bureau of Primary Care Services is to improve the health ofGuam’s community by empowering patients to be responsible for their health and offering quality patient care.
